Avera Marshall Social Worker Designee - Long Term Care in Yankton, South Dakota
Job Summary:
The Social Services Designee (SSD) is a blended role with primary responsibilities in social services.
Full time benefit eligible position; day hours; Every 5th weekend/holiday rotation; 80hrs/2wks
A primary purpose of the position is to enhance the resident's quality of life working closely with the RN Supervisor effectively managing the household with guidance from a Social Service Mentor.
Participates in the admission decision process, transfers, room changes and discharge arrangements.
Develops positive resident relationships, exploring approaches to aging and supporting families.
Participates in care conferences for all household residents. Communicates care conference results with staff.
Position Requirements:
The SSD must have a minimum of 2-3 years experience in long-term care, Associate of Arts Degree or Bachelors Degree in related field preferred.
Experiences in social services are preferred.
The SSD must demonstrate leadership abilities to develop and motivate people; possess good critical thinking and problem solving skills; exhibit the ability to prioritize responsibilities; be flexible and able to work under stressful situations; possess good verbal and written communication skills (English); be self-directed and work autonomously
Certified through the State of South Dakota, for Nursing Assistant and/or is eligible within 1 year of hire.
BLS certification is required.
